My dad spent about two months here after a broken hip. The staff were very caring and responsive and wanted to support. All the people in OT and PT as well as all the nurses were wonderful. Thank you, you made a big difference to our family and helped my dad get back on his feet.

My dad was recently at Shadelands for close to 3 months. I was extremely pleased with the care he received, the staff, and with the facility itself. A few points: 1) The building is so nice! It’s modern and cheery. And the courtyard area, which my dad could access from his room, was beautiful and a nice retreat when the sun was out. 2) My dad’s room was great too. Lots of natural light, high ceilings, a tv that was easy to work, with all the channels he needed (this may seem small, but the cable situation at another SNF was terrible and my dad was perpetually frustrated). Also, the fact that Shadelands only has single rooms is so nice. 3) I was impressed by the cleanliness of the facility. I often saw the cleaning people when I was there, and I could see that they were doing a fantastic job. 4) Every staff member we met, from the directors, to the front desk receptionist, to cleaning staff, was friendly, caring, and welcoming. I sensed that the staff was happy at Shadelands. 5) Aman and her rehab crew were excellent! I talked to Aman several times – she was always receptive, informative, and helpful. And her staff did a great job with my dad. 6) Cristina, the Social Services director, was also great! I talked to her multiple times about the plans for my dad. She was always so helpful and so friendly. 7) I’m so impressed by the communication from Shadlelands staff. From the introductory meeting to the check in conversations, to the follow up calls I’ve received after my dad’s departure, I can see that communication is a point of emphasis, and one that I very much appreciate. 8) The food was really good, something I didn’t think I’d say about a SNF 😀! And it was great that they offered alternatives for meals (like sandwiches instead of hot meals for lunch or dinner). 9) Special shout out to a few staff members who I saw frequently while I was there: Fay (front desk), Lesley (RN), Jewel (CNA), and Marcella (CNA). All were so friendly and so caring to my dad!

Very recently, I fell and broke my hip – a non-displaced fracture of the femoral neck. After a successful repair procedure (aka surgery), which included installation of three metal pins (which when x-rayed look like three wood screws) was off to Shadelands Post Acute, for recovery and therapy. Was, almost of course, leery of being packed off to what, I thought, must be the equivalent of a nursing home; nowadays called a skilled nursing facility or SNIFF in the biz.(Note to Google – time to update your category name for this type of place) In fact, being here has been a kind of pleasure. Building is new-ish and up to date, all private rooms, gym used for physical therapy. Very unexpectedly, even the meals are good! However, the real high points are the staff, who are simultaneously expert in the range of roles and, somehow upbeat, positive and continually helpful, something much appreciated by us invalids. While there are too many to list here, I want to mention Judith Don, Herna Adon and Babylyn. To all of you, thank you! P.S. Wrote the foregoing review when it seemed I might be on the verge of discharge. (As I mentioned to the staff then, as much as I like Shadelands Post Acute, will be happy to go home. Developed, almost of course, that my optimism over an imminent discharge was a bit too early. Several weeks later, though, and I was on my way, my high opinion of the place undiminished. In those last weeks, the staff continued their considerate attentions to this then invalid. Realized it wouldn’t right, then, not to mention several who were especially helpful, including CNA Dominique and LVN Jeremiah. Must mention, too, Agnes, whose lively, intelligent, personable ways were an elixir for this old man.

My client was in this facility for 2 months and I came and visit her every day and I must say she loved the CNA’s especially Kaitlin and Alessa, some PT’s and OT’s were great except this one PT Mae is terrible and awful, first time I observed her doing therapy with my client she just left her after doing 1 exercise and then she went talking, on her phone and eating with her peers while my client’s wondering and feeling helpless on what to do next, the second time my client had PT with Mae was that she was doing 2 patients in one setting, leaving 2 people supposedly doing their exercises and she’s off somewhere doing something else, what a terrible service. I’m hoping that anybody’s love ones who’s staying here will not get a therapy from this person. It’s too bad that this place is wonderful and have this person working here who doesn’t deserve to be here and making this place looks bad with her terrible service.

It’s often challenging to get needs met quickly in a convalescent care setting, so I want to give a shout out to Emily and Wendylen, nurses at Walnut Creek’s Shadelands Post-Acute. My adult daughter, who has Down syndrome and many health issues, was in residence there recovering after a fall precipitated a long hospitalization. Physical therapy being terribly hard for her, I decided to get an orthopedic consult to assess her risk/benefit. I was offered appointments at their nearby offices a month out, or…only about 90 minutes after my call! I needed the information they could provide asap so I opted to get her there any way I could. That’s where Emily and Wendylen came in. They changed her quickly and dressed her for the cool day outside. They modified her wheelchair with elevated leg supports so the blisters on her shins would be less likely to flow. They brought in the equipment to help her stand for transfer to the chair and secured her once she’d moved. All that while being cheerful! No lie, I would’ve loved a transport van to have been an option, and I’m too old to have pushed her half a mile, but the rapidity with which Emily and Wendylen helped me, no questions asked, made it possible for her to arrive at the physician’s office in time, where I got my valuable information. And when we returned, they made sure that we got some lunch!

My wife recently stayed at Shadelands for about 47 days. I had the pleasure of visiting her daily and was always greeted by name in a friendly and welcoming manner. Feh and Marj at reception were so welcoming! The staff is the key. They are all wonderfully kind and supportive! The care was great! An additional mention and thanks to Ariel for all his kindness and support of my wife.

I have a family member staying there and we were told to stop asking questions or she would be kicked out of the facility. This is a scary time for us and just trying to process, and asking questions is how we do so.

Business response 56 days ago

We are disappointed to hear about this concerning experience and understand how distressing this situation must be for you and your family. We recognize that having a family member in our care is already a challenging and emotional time, and asking questions about their well-being and care is not only natural but also your right as a family member. We take your concerns very seriously and want to ensure that all families feel comfortable and supported when communicating with our staff about their loved one’s care. We would like to hear more about this specific incident so we can thoroughly look into what happened and address any issues that may have occurred. Please contact us at (925) 448-3782 to discuss your family member’s care and this concerning interaction you experienced with our facility.

My husband got ill recently. And we were both so thankful that he was able to go to Shadelands for his care. Shadeland has a very happy positive environment. They are not just interested in the physical care, but also the emotional care of their patients. They provide music and activities so that they can keep the spirits of their patients up. THANK YOU ALL AT SHADELANDS! Special thanks to: Marge: she always greeted me like a friend. if I had any questions, she knew the answer or directed me to the person who could answer my question. She was also very helpful small things like one day I bought my husband sushi and we needed soy sauce. I knew if anyone could help me it would be Marge. Sure enough she had an extra packet and shared. Melanie: very conscientious nurse kind caring and very efficient. Nora: She too was kind and showed personal interested and was very informative. JJ: was a nurse who was very professional and helpful. Dolores: was efficient, accommodating, and had a fun spirit. Anna; was accommodating, sweet, gentle and will spirit. Kristen: was the dietitian and was very caring, attentive, helpful and respectful to my husband‘s emotional concerns as well as his diet. Caleb: was helpful, challenging and focused as a physical therapist. Kashvi: as a occupational therapist she was very dedicated and serious. Christian: was available and very efficient in answering us with all our questions. Lydia: was joyful, happy and good communicator. Meetel: extremely efficient and would stop doing what she was doing and would give me her full attention with my concern. I also really love the fact that the place was kept very very, very clean.

Shadelands Post Acute does a great job taking care of their residents. No skilled nursing facility is ever perfect, but this team truly strives to provide good service and support families the best they can. You can tell they genuinely care about the people they serve. Cristina Dela Cruz, the Social Services Director, really stands out. She stays on top of her discharges and makes sure residents leave with a solid, safe plan in place. What I appreciate most is that she doesn’t just hand them off and move on — she follows up even after they’ve left to make sure they’re doing well. That kind of follow-through and heart makes a big difference.

Don’t waste your time!!! The place looks nice like FAKE (AI) picture , the staff and care is horrible!!! My grandmother was there for a short time u til I removed her. I was worried every single moment she was there because they didn’t have anything right, the medication was wrong, they left her in her room for hours with no assistance. My grandmother couldn’t walk she could barely talk and couldn’t feed herself. She needed MORE and they couldn’t provide that for her. She would ring the bell and NO ONE came until I called the front desk asking to go and help her. The only good thing about that place was the speech therapist Kathleen and that woman deserves a raise!

Three months ago I became gravely ill due to my lupus and had I to spend a month in Shadelands to regain my strength to prepare for a upcoming surgery in November. I was very apprehensive about going into a skilled care facility, post acute, “aka” nursing home. Shadelands is in a class of its own. I never once felt like I was a patient. I felt like a guest at a resort. The entire staff and I mean the entire staff are very caring and attentive. The nursing staff is very detailed oriented especially when giving your medication, they tell which medication they are giving you each time also upon admission you are given a printed list of the medication with the dosage you be receiving to for you to review for accuracy to go over with the medical doctor who is the best Dr. Tiffany King. Your assigned CNAs consistently checks in to see if you need anything from assistance with bathing making up your bed, bring your meals to room or dining in the common area, whether if your water needed to be refilled or just to say hello. Management checks in religiously every morning to make sure everything is to your satisfaction. The facility is maintained to a pristine condition. The Physical and Occupational Therapy Staff are outstanding and very patient and encouraging in getting you to your recovery goal. The dietary menu is excellent which includes additional options to chose from with an added plus to purchase meals for your guest to dine with you. Additionally, there is activities coordinator that schedules a variety of daily activities including weekly musical entertainment as coordinated weekly spiritual services with communion on Sunday. The Activities Coordinator can also arrange for beauty services from hair services including manicure/pedicure. The large private en-suite rooms with private bathrooms with Direct TV are definitely also a major plus. Furthermore, daily housekeeping and janitorial services are present throughout day. As I am writing this review I had my surgery and now I am back for my long recovery and I consider Shadeland’s staff my family and they welcomed me back with open arms and that’s important. I trust Shadelands with my life when it comes to my medical care in particular because I also have so many underlying critical conditions besides lupus that I I trust wholeheartedly that Shadelands are beyond qualified with there premier medical staff to handle my care that’s why I returned to continue to my long-term recovery. Currently, I am a lupus advocate, a spokesperson for a major pharmaceutical company and a retired publicist having worked with some of the countries top celebrities and many of my former clients have gone to post acute facilities for various reasons. Since seeking aftercare at Shadelands I consider it one of the countries best post acute facilities that I have experienced both professionally and personally. I would highly recommend Shadelands as the preferred post acute facility to seek rehabilitation in California especially in Northern California. This facility deserves an infinite amount of stars. Shadelands should be the blueprint on how post acute should be modeled afterwards.

I’ve been fighting for 13 days trying to get the medications for my patient that supposedly were called into the pharmacy the day before he left. I’ve made over 20 calls and told ten times they were in the pharmacy. Still nothing there. I have never seen such incompetence and I would never use this facility. They can’t even call you back when they tell you they will even though the medications are crucial to get.

I came to Shadelands Post acute on May 27 and discharged July 18,2025. I had a pelvic fracture which required an external fixator which staff at the facility had never seen before. It required regular attention with cleaning the posts. JJ was the wound nurse that worked diligently with me even when I ran into complications with it. Helped me through a lot. I can’t say enough about the care I received here. From the kitchen, nursing staff , prompt response from the cnas, activities with fantastic musician Cesar!! Arcel the activity director was exceptional, full of fun and energy! And brought me ice tea everyday! The OT/ Pt Dept was wonderful.. gave me challenges that I didn’t even know I could do! They enabled me to walk out on discharge without even a cane! Such a recovery! Can’t thank them enough! The maintenance people kept the place spotless. Best of all I had Room 24 .. the best room in the place since it had a door to the inner courtyard and I could sit there through the day and enjoy the sunshine ..was perfect for my healing. Thank you Shadelands post acute fir such a great experience and recovery!

I cannot say enough good things about Shadelands. The room was comfortable, well lit from a big window and kept clean at all times. The administrative team takes great care of all tasks surrounding running Shadelands well. I can’t begin to list all the support people who took direct care of me! The nurses, the wound care nurses, the CNA’s who were my lifeline for over three months, the physical and occupational therapists. Because of all of these people I be am ready to go home!

I highly recommend Shadelands Post Acute. My mother had an extended stay at Shadelands and was very pleased with the care and compassion of the entire staff. The facility is fairly new and is very clean and well organized for the care and rehab of patients. Everyone from the front office staff, nursing staff, doctors and the maintenance personnel were friendly and caring. The nursing staff were very attentive and all of the CNA’s were fantastic. Shout out to the CNAs Trina, Zanita and Erica who were very caring and professional in their care of my mother. The physical therapy team (PT and OT) is excellent and the physical therapy room is well equipped. The food is fresh and of high quality and attentive to special requests. There are scheduled activities like musicians who perform for the patients.

I was a patient at Shadelands Pist Acute for three months, from January to April 2025. I have nothing but praise for the Shadelands facility. It is beautiful, well maintained, and comfortable. The staff members are all kind and caring people. I will start with the therapy personnel, the CNAs, the wound care personnel, the LVNs/RNs, the transporters, the activity directors and lastly, the laundry and cleaning staff. They are all attentive, kind and caring. I actually enjoyed my stay at there, and would return there if I need a facility again. Wonderful facility!

A family member was a patient for 6 weeks at Shadelands. This postacute care facility is top notch. A beautiful building, delicious food and most importantly, the nursing staff, the PTs, the OTs and doctors were attentive, knowledgeable and caring. There were also a number of fun activities such as chair yoga, floral arranging, bingo and musical performances too. A care facility with an enlivened atmosphere as your loved one gets strong enough to return home.

I had a good experience at Shadelands. The large, private rooms, each with an attached bathroom are nice. The staff who provided the services, whether the nurses, the CNAs, or others, were friendly and helpful and responded quickly when I pressed my call button. The physical and occupational therapists were very good. The only criticism I would have is that I had different PT and OT therapists each day for my relatively short stay of only five days. It would have been beneficial I think to have more continuity in that area. But overall, my experience at Shadelands was very positive.
